VOLUNTARY SAFETY RECALL OF CABLEMOD 12VHPWR ANGLED ADAPTERS, V1.0 and V1.1

VOLUNTARY SAFETY RECALL

CableMod 12VHPWR ANGLED ADAPTERS V1.0 and V1.1,
90 Degrees and 180 Degrees, Variants A and B

We are voluntarily recalling all of our angled adapters to keep our customers and their hardware safe.  We have found that the male connectors on the adapters could become loose during cable management and system use, overheat, and melt into the GPU, posing fire and burn risks. We encourage all of our customers to participate. This recall applies only to our angled adapters and does not affect our angled cables.

We are recalling all models of our angled adapters:

CableMod 12VHPWR Angled Adapter V1.0

90 Degree V1.0 Angled Adapter A/B

90 Degree Variant A 90 Degree Variant B

180 Degree V1.0 Angled Adapter A/B

What We’re Doing for Our Customers:

We are offering customers the choice of either a full refund, including shipping, or a $60 / 60€ store credit to be used for any non-customized product that we sell, with free standard shipping. You will receive a full refund or store credit for each V1.0 and/or V1.1 angled adapter that you purchased.

If you elected to receive a V1.1 angled adapter through our Early Adopter Program, we will also be offering the choice of either a full refund or store credit for the shipping amount. You will not receive a refund or store credit for the V1.1 angled adapter that you received for free.

Only one type of remedy can be selected for this recall: either all refund or all store credit.

You must have a PayPal account if you elect to receive a refund.

How to Safely Remove the Angled Adapter from the GPU:

To ensure your safety, please follow these step-by-step instructions on how to remove the angled adapter from your GPU.

  1. Please do not touch the adapter while your system is running.
  2. Power down your system and wait until the adapter has had adequate time to cool down before handling.
  3. When the adapter is cool to the touch, depress the safety clip with your finger while gently pulling the adapter from the GPU.
  4. If you have the 180 Degree, Variant A adapter, you will need to use a spudger, popsicle stick, or other firm but flexible instrument that can fit between the adapter and GPU to depress the safety clip while gently pulling the adapter from the GPU. CableMod discourages use of a flathead screwdriver or other rigid instrument because of the possibility of damaging the GPU.
  5. If the adapter will not disengage from the GPU using reasonable force, the angled adapter may have failed. Please contact customer service for assistance. Support – CableMod or toll-free at 888-203-3605, Monday through Friday, from 9:00 a.m. to 5:00 p.m. (ET). If your GPU is damaged because of a failed adapter, we will repair it if your damage claim is submitted to us by March 7, 2024. Please note that CableMod is no longer reimbursing the purchase price of a damaged GPU.

FREQUENTLY ASKED QUESTIONS

What is a voluntary recall? Am I required to participate?
A voluntary recall happens when a company, like CableMod, takes the initiative to remove a product from the market to prevent a risk of harm to consumers or their property from its continued use. We have worked in cooperation with the U.S. Consumer Product Safety Commission to design a process that will help us do just that with our angled adapter, while providing a remedy for our loyal customers. We can’t force you to participate in this recall, but we strongly encourage you to do so because of the risks of physical or property damage posed if an angled adapter fails.

This recall applies only to the CableMod 12VHPWR Angled Adapters, V1.0 and V1.1 models. They come in 90 Degree and 180 Degree versions, Variants A and B. It does not apply to our angled cables.

My angled adapter still works. Can I keep using it?
No, you should not. The male connector on the angled adapter can become loose, overheat, and melt into the GPU, posing fire and burn risks. Stop using the adapter immediately. Please do not touch the adapter while the system is running. Power down your system and wait until the adapter has had adequate time to cool down before touching it. Please consult our instructions on to how to safely remove the angled adapter from the GPU.
I purchased more than one angled adapter. Will I receive a cash refund or store credit for each one?
Absolutely!

Please note that if you participated in our Early Adapter Program, you will not receive a refund or store credit for the V1.1 angled adapter that you received for free.
I received my V1.1 Angled adapter for free under the Early Adapter Program. What are my remedies?
If you participated in our Early Adaptor Program, you will have the choice of either a refund or store credit in the amount of your shipping cost. You will not receive a refund or store credit for the V1.1 angled adapter that you received for free.
I PAID FOR CABLEMOD’S EXPRESS SERVICE. DO I GET REIMBURSED FOR THAT?
No. CableMod’s Express Service is just that – a service. For a fee, it allows your order to be placed at the front of the queue for processing. It does not expedite shipping itself.
I have purchased multiple adapters with more than one order. Can I submit one claim for all orders?
No. You will need to submit a separate claim for each order number so that your refund(s) can be processed accurately.
I didn’t purchase my angled adapter directly from CableMod. Can I still get a refund or store credit?
Yes, but you’ll need to do the following things:
  1. Select the authorized online retailer from which you purchased your adapter on the recall form.
  2. Provide your Order Number.
  3. Upload your receipt/invoice for the purchase. Check your old emails, as many online retailers send receipts and proofs of purchase that way.
  4. Make sure that you have a PayPal account if you want a refund.
Please note that all recall claims based on purchases other than through CableMod’s online stores need to be separately verified by the relevant third-party retailer. It may take longer to process your claim because of that.
Is Amazon considered a third-party retailer?
Yes, it is. You will need to follow the procedures for submitting a claim for angled adapters purchased from authorized third-party online retailers.
I received my angled adapter as a gift? What should I do?
We will need to know the name of the person who gave you the gift and where they purchased it, as well as which angled adapter model you own. That should give us enough information to verify our records, or those of our authorized online retailers. Please call us toll free at 888-203-3605 from 9 a.m. to 5 p.m. ET Monday through Friday, or reach out to customer service at support@cablemod.com with that information. We’ll take it from there.
I purchased my angled adapter second-hand on E-bay? What should I do?
Please call us toll free at 888-203-3605 from 9 a.m. to 5 p.m. ET Monday through Friday, or reach out to customer service at support@cablemod.com. We’ll be happy to help.
I didn’t purchase my angled adapter directly from CableMod. Can I get a refund or store credit from my place of purchase?
No. All claims under this recall must be submitted to CableMod.
I threw away my angled adapter. Am I still eligible for a refund or store credit?
Yes, but you’ll have to contact customer support to submit your recall claim. Customer support will provide you with a “Missing Adapter Declaration” form, a separate Claim Verification Code, and a special link dedicated to submitting claims involving missing adapters. Once you submit the required documentation, your claim will be processed.
What’s the "MISSING ADAPTER DECLARATION" form? Why do i need to submit it IF I'VE ALREADY THROWN MY ADAPTER AWAY?
The point of a recall is to remove a product from the market to prevent a risk of harm to consumers or their property from its continued use. We are required to have proof that an angled adapter has been taken off the market as a condition to processing a recall claim. When a photo of a destroyed angled adapter can’t be submitted, the Missing Adapter Declaration is used in its place. In it, the customer certifies that (s)he can’t provide a picture of a destroyed adapter because (s)he has thrown it away.

Again, our customer support will provide you with a “Missing Adapter Declaration” form, a separate Claim Verification Code, and a special link dedicated to submitting claims involving missing adapters. Once you submit the required documentation, your claim will be processed.
I want a refund. What should I do?
You must have a PayPal account to receive your cash refund. If you do not have one, you’ll need to set one up before you submit a refund claim. You will also need to provide an email address linked to your PayPal account in order for us to process your refund.

Once CableMod verifies your claim, it will issue a cash refund to your PayPal account. We will send you an email to let you know that we have verified your claim and have issued a cash refund through PayPal. You will also receive an email from PayPal to let you know that there is money in your PayPal account that you must claim. If you do not have a PayPal account, you will not be able to access your cash refund, and it will be returned to us.
I purchased my angled adapters directly from CableMod using my credit card. Can I receive my refund directly to that card?
Unfortunately, no. We are only issuing cash refunds through PayPal for this recall.
I want a refund but don’t have a PayPal account. What should I do?
You will need to set up a PayPal account before you submit your claim for a recall. You will need to provide an email address linked to your PayPal account in order for us to process your refund.

If you do not have a PayPal account, you will not be able to access your cash refund.
How do I use my store credit?
After your recall claim has been verified, you will receive a gift card code to use when you go through checkout on our webstore. The coupon can only be applied against non-customized products. Remember, standard shipping is free!
How long will it take to process my store credit or refund?
We anticipate that the volume of claims will be very high at the beginning of the recall, which may affect processing times. Since we’re a small company, please allow 6-8 weeks for processing. Verifying claims from third-party retailers may also require additional time. We’ll do our best to process claims as quickly as possible.
Is there an end date for the recall program?
At present, no. Our goal is to get the adapters off the market as quickly as possible. Please make sure that you are not using your adapter and submit your claim as soon as you can.
I've submitted a claim, but then realized that I made a mistake on the form. What should I do?
If you made an error, simply fill out the form again. We will only consider the newest submitted form.
I participated in the Early Adopter Program but chose to receive an angled cable. Do I get a refund or store credit for my shipping?
No. This recall is for our angled adapters only.
What is/was the deadline for submitting a repair claim for a damaged GPU?
Unfortunately, March 7, 2024 was the deadline for submitting a repair claim for GPUs that may have been damaged by a faulty angled adapter. We will not be honoring repair claims made after that date.
What if I didn’t receive a notice of the repair claim deadline?
CableMod has repeatedly provided its customers with notice of the deadline for submitting repair claims for damaged GPUs, both in writing and on social media.
Where did you post notice of the deadline for repair claims?
CableMod has posted the deadline for repair claims in numerous places:
  • on its website and recall webpage;
  • through two direct email notices to CableMod customers;
  • through direct email notices from third-party retailers;
  • through its toll-free hotline;
  • on social media, including Instagram, Facebook, and Reddit;
  • through press reports published in computer-focused media; and
  • through influencers of social media.
I didn’t have time to submit my repair claim. Can I still submit one?
No. We believe that our customers have had an extended period of time to submit any repair claims for damaged GPUs.
  • Beginning last summer, we have offered assistance to anyone who was affected by a failed adapter.
  • We have also advised customer to stop using the angled adapters immediately in several notices during this recall process, beginning on December 22, 2023. Both the U.S. Consumer Product Safety Commission and Health Canada have also provided the same directive in public press releases and on their websites. Any damaged GPU would have been identified if the angled adapters had been removed on or before March 7, 2024.
  • We have also provided 4 weeks’ notice of the repair claim deadline in public notices that are identified above.
I have a special circumstance that prevented me from submitting my repair claim on time. Will CableMod still consider it?
CableMod will be strictly enforcing its March 7, 2024 deadline for submitting repair claims. If you believe that you have a special circumstance that CableMod should consider, please send an email to customer service at cablemod.com/support/ and explain your circumstance. While it is unlikely that we will make exceptions to our deadline, we will nevertheless review your special request and get back to you.
